Appointments

You do not need a referral to schedule an appointment, unless required by your insurance carrier. Check with your carrier to make sure your EvergreenHealth primary care provider is in your network.

Same day appointments

If you need a same-day appointment, we ask that you call the office. If your doctor is not available, you may have the option to see another provider the same day. Let our team know if you cannot wait until the end of the day and the best phone number for us to reach you.

Virtual visits

MyChart Patient Portal

Log into MyChart to find your health information online or to email your provider. Learn more about the benefits of the patient portal and how to register.

All patients over the age of 13 may create a portal account either in person or online.

After-hours care

If you have a question about a non-emergency health concern when our office is closed, call our office phone number and you'll be transferred to the answering service. If the question is of an emergent nature, the after hours provider may be reached.

Late arrivals

If you arrive at the clinic 10 (or more) minutes late for your appointment, you may need to be rescheduled. The front office staff will see if anything in the provider’s schedule has changed to allow you to be seen. If not, you will be asked to reschedule.

If your appointment is for acute symptoms or something that cannot wait, the front office team will check schedules of other providers at that clinic location, providers at other primary care clinic locations, or suggest Urgent Care.

Appointment cancellations

Provide us with at least a 24-hour notice if you are unable to keep your appointment. This allows us to make the time slot available for other patients.

When should I arrive for my appointment?

If you are a new patient, arrive 15 minutes before your appointment.

What should I bring?

For each visit be sure to bring:

  • Complete and submit all of the Clipboard forms you have received on your MyChart patient portal. You may also have additional forms to complete at check-in.
  • Copay
  • Insurance card
  • Medical records that relate to your visit (if available)
  • Medication list or containers (especially for new patients)
  • Photo identification
  • Mask

Lab tests

Many of our primary care clinics offer onsite laboratories. Our clinic staff will give you the best lab option the suits your needs. Your lab results will be posted to your MyChart patient portal, and clinic staff will also reach out with a phone call or letter.

Prescription refills

When you need a medication refill, call your pharmacy. Plan ahead since refill requests take two business days.

When you are requesting a new prescription, or have no refill available, plan on making an appointment with your providers.

Rest assured knowing your primary care provider will coordinate your care with a network of specialists at EvergreenHealth if needed.

Quality survey

Following your visit, you may receive a quality survey by email or by US mail from Press Ganey (a research firm). Patients are randomly selected. If you receive a survey, and complete it, we’ll use your feedback to help provide excellent patient care.
