Community Healthcare Access Team (CHAT)
If you are having trouble accessing affordable and quality health and social services, we can help.
We're the Community Healthcare Access Team (CHAT), and we partner with many community organizations to help low income and uninsured residents of EvergreenHealth's service district.
Our programs are supported by tax levy funds, and serve residents living in cities within King County Public Hospital District No. 2.
Access to Insurance
If you're looking for insurance coverage, CHAT staff are specially trained in Medicaid and Medicare eligibility, and are certified as navigators for the WA Healthplanfinder (aka The Marketplace or The Exchange). They are also knowledgeable about various community resources.
Health Care Referrals
One of the team's primary roles is to assess, coordinate and make appropriate referrals for your care within and outside the EvergreenHealth network.
CHAT also addresses barriers to services such as:
- limited financial resources and/or social support
- lack of health insurance and access to quality care
- limited language proficiency
Some of the services CHAT can provide include:
- assistance in accessing charity care
- pharmaceutical patient assistance programs
- referrals to transportation programs
- psycho-social assistance
Community Outreach and Education
If you're dealing with issues that impact your quality of life, such as postpartum mood disorders, Medicare, senior housing options and advance care planning, CHAT staff can provide written materials and facilitate presentations, workshops and support groups on a variety of topics.
Social and Health Community Services Connections
CHAT partners with community agencies to bridge gaps in service delivery and improve access for clients.
These partners include the Lake Washington School District, Project Access NW, local Senior Centers (Kirkland, Redmond, Northshore, Kenmore, and Mill Creek), Hopelink, local food bank and Perinatal Support Washington.
