Online Pre-Registration
If you have already made your appointment, you can save time by using our online pre-registration form.
This pre-registration form is for the following services only:
- Maternity (use your expected due date)
- Surgery (Blue only)
- Diagnostic Imaging
- Comprehensive Procedures
All other services require in-person registration at the time of your appointment.
Please note:
- This is for first time patients only
- The online pre-admission form must be completed at least 72 business hours prior to your scheduled appointment.
- If you are within 72 hours, please contact our staff at (425) 899-2722 to complete your pre-registration.
