Insurance Plans & Coverage
Does my insurance cover care at EvergreenHealth?
Before you schedule any medical services, you want to make sure these services are covered by your health insurance policy.
These links list the insurance carriers that cover EvergreenHealth clinics and services, our employed providers and the medical center.
Because some carriers offer multiple plans, it's possible that your specific plan might not include care at EvergreenHealth. To be sure, please contact your insurance carrier; they know the specifics of your coverage, plan, deductibles and co-pays.

Get a Cost Estimate
Your health insurance provider can provide an estimate of what you will pay for a specific procedure, including your out-of-pocket costs, because it knows the specifics of your coverage, plan, deductibles and co-pays.
Some insurers have online tools and estimators, while others will ask you to call and speak to someone.
