Please bring your insurance card to your first appointment, and expect to pay a co-pay if applicable.
Cancellation Policy
We request 24-hours notice if you need to cancel your appointment. We will be happy to reschedule your appointment to a time that is more convenient for you.
Cost Estimates
We are happy to provide an estimate of charges for the services we provide. These estimates are inexact as we cannot always know in advance the actual services your provider will determine you need during your visit. Your charges will be based on the actual services you received.
Billing
As an outpatient department of EvergreenHealth, we bill both a professional fee (for the physician and practitioner services) and a facility fee (for costs such as supplies and staffing), so you will receive two separate bills for each visit or procedure at the EvergreenHealth Sleep Disorders Center. As a result, you may pay more for certain services than you would at a free-standing clinic. For any questions related to your facility fees, please contact 425.899.1600. For questions related to your professional fees, please contact (425) 899-3292.
If you have Medicare: We accept Medicare assignment and will bill your Medicare and Medicare supplemental/secondary insurance for you. Most patients with Medicare are responsible only for their usual 20% co-pay, and/or any unmet deductibles. If you have questions about services that Medicare does not cover, please contact Medicare directly at 1-800-MEDICARE.
If you have commercial insurance: We will submit separate claims for the facility and the professional services to your insurance company. Dependent upon your benefits, you may be responsible for separate co-payments. If you have questions about your coverage please contact your insurance company directly to see what your policy will pay and what out-of-pocket amounts you may have to pay.
