How Concierge Medicine Works

What is concierge medicine?

Concierge medicine is a different approach to primary care where patients pay a monthly fee for significantly greater personalized access to a physician and other individualized services.

In the case of EvergreenHealth Signature Care, patients may schedule appointments for the same day, or next day, for acute needs. Your physician is available via phone 24/7 and will respond to your emails within 24 hours.

Other advantages include flexible appointment lengths based on your needs. Our doctors are not rushed to get to the next appointment. They can also be available after hours – or even make home visits when necessary.

The membership fee also includes appointments with a health and wellness coach who will work with you to develop a comprehensive, personalized wellness program based on your needs.

Who should enroll in EvergreenHealth Signature Care?

EvergreenHealth Signature Care is available to anyone who would like a more personalized relationship with their primary care physician and enhanced access for questions or consultations.

People with chronic illnesses or health concerns like the longer appointments and the higher level of involvement with their physician.

People interested in disease prevention and wellness appreciate the added benefits of appointments with our nutritionist and our exercise physiologist.

Busy people like the ease of scheduling appointments around their business or travel schedules. They can even contact their doctor by phone, text or email while traveling.

Families benefit by getting fast answers to health questions any time of day or night. The entire family can get their appointments scheduled together, and it’s also easy to get in quickly for office appointments when an illness pops up.

Is the monthly Signature Care fee tax deductible?

Your tax advisor can clarify whether the fee qualifies for any tax-advantage.

Do I have to sign a long-term contract?

No. There is no long-term contract. While we try to meet and exceed the needs of our patients, you can provide a 30 day notice of termination at any time for any reason and if applicable, we will prorate your fee at the time of termination.

Is there a limit to the number of patients EvergreenHealth Signature Care physicians take on?

Yes. Low patient-to-doctor ratios are a unique feature offered by EvergreenHealth Signature Care. Our physicians care for a limited number of patients; once that limit is reached, the doctor is no longer available and a new physician is added to ensure every patient receives personalized service.

What are the office hours?

Our office is open weekdays from 8 a.m.- 5 p.m., and after hours appointments are available depending on need.

Signature Care members always have 24/7 access to their physician via phone or email.
