Hypertension or high blood pressure is one of the most common reasons for visits to providers' offices.

Hypertension is dangerous because there are usually no symptoms, but if left untreated over time, it can increase the risk of serious health conditions such as heart disease and stroke, the leading causes of death in the United States.

Your provider may refer you to EvergreenHealth Pharmacist Provider Services to help manage your blood pressure.

Our Approach

Diet and Exercise. The foundation for treatment of hypertension. Your pharmacist will provide diet, exercise, and other lifestyle recommendations to help reach your blood pressure goals.

Medication Management. Sometimes diet and exercise alone are not enough to properly control your blood pressure. Medications are often required in order to get your blood pressure to goal.

Your pharmacist will work with you to select an appropriate anti-hypertensive medication plan. Through follow-up visits, your pharmacist will closely monitor the medication prescribed to ensure it's the best fit for you.

How to Get Started

Your physician will need to send a referral to enroll in hypertension management offered by EvergreenHealth Pharmacist Provider Services.

Your visit is typically covered by insurance, but you may want to verify this coverage with your individual health plan.

Once we have received a referral from your physician, we will contact you to schedule your first appointment.
