Hospice services may be appropriate for individuals with a terminal diagnosis or at the end stages of a chronic disease. If you have noticed a decline in your patient or resident's condition, it may be time for a Hospice assessment.

EvergreenHealth Hospice Care provides compassionate, patient-led care for individuals nearing the end of life across King and Snohomish Counties. Services are provided wherever the person resides including independent and assisted living communities and adult family homes.

To place a hospice referral:

  • EvergreenHealth providers may place a hospice referral directly into Epic
  • Call EvergreenHealth Hospice Intake to speak with a nurse: (425) 899-1020
  • Fax referral and relevant clinical information to EvergreenHealth Hospice Intake: (425) 899-1033

Fax the following (as available) to (425) 899-1033:

  • Completed Hospice Referral Form (links above)
  • Resident Demographic "Face Sheet"
  • History & Physical
  • Most current provider progress note
  • Medication list and/or care orders
  • Weight summary (ideally 6+ months)
  • POLST forms
  • Name of primary provider

Medicare, Medicaid, and most private insurance plans provide a hospice care benefit, often with little-to-no out of pocket cost.
