To place a hospice referral:

- Completed Hospice Referral Form (links above)
- Resident Demographic "Face Sheet"
- History & Physical
- Most current provider progress note
- Medication list and/or care orders
- Weight summary (ideally 6+ months)
- POLST forms
- Name of primary provider
Medicare, Medicaid, and most private insurance plans provide a hospice care benefit, often with little-to-no out of pocket cost.
