Home Health Services
EvergreenHealth Home Health provides the following options for care and services:
Nursing Care assesses effectiveness of medications and treatment for specific illnesses, including pain control, wound treatment, diabetic management and catheter care. We also provide patient/family education to maximize wellness and self-help.
Physical and Occupational Therapy can maximize independence through safety assessments and home modifications along with strengthening and home exercise programs to assist in recovery from physical limitations due to surgery or illness.
Speech Therapy and language rehabilitation assess and treat swallowing and speech difficulties as well as problems with memory and judgment.
Social Work offers assessment and counseling regarding financial and support needs along with referrals to community resources and planning for long-term care.
Home Health Aides provide assistance with bathing and personal hygiene.
Caregiver Support
Seamless transitions to Palliative Medicine, Hospice and other home-based programs.
