Our patients enjoy the convenience of the EvergreenHealth Heart Care patient portal (not part of the MyChart Patient Portal).

If You Are a First Time Patient

Create a MyChart account, then complete your registration form.

You will receive an e-mail from us that includes your user ID and a link back to our patient portal.

You may log in at any time to begin using our services.

If You Are a Returning Patient

Log into MyChart to:

• Request appointments
• Send and receive messages
• Renew prescription
• Access health forms

Insurance Information

We accept most insurance plans. Check with your insurance carrier to make sure EvergreenHealth Heart Care is covered by your policy.

Payment is expected at the time of service unless prior arrangements have been made with the billing department.
