Cardiac Rehabilitation
Cardiac Rehabilitation is a comprehensive outpatient program designed to maximize your recovery and help you resume an active and heart-healthy lifestyle.
Our experienced cardiac rehab team, along with your physician, provides the support and guidance to assist you on your journey back to better health.
Our Program
A typical program lasts 8-12 weeks. You are assigned a case manager who establishes an individualized program based on your input and health needs.
This program is for individuals who have experienced:
- Heart attack
- Coronary bypass surgery
- Angioplasty, stent placement or atherectomy
- Heart valve surgery
- Angina
- Congestive heart failure
- Other heart related procedures or problems
The program will help you:
- Learn to manage your risk factors
- Improve efficiency of the heart and muscles
- Develop safe exercise habits
- Return to your previous level of activity
- Reduce anxiety and depression
- Gain camaraderie through group participation
- Learn to manage your health with respect to your heart condition
- Increase your strength and stamina
Program Components
Medical
Upon enrollment, your case manager will complete an initial evaluation, and along with our Medical Director and your physician, and will develop a treatment plan to meet your needs and interests.
Throughout your participation, your case manager will provide feedback on your progress and also report any concerns to your physician. Progress reports will be sent to your physician periodically during your participation.
Exercise
You will participate in group exercise classes, but at your own level and rate of progression. The classes include aerobic and strength training as well as relaxation exercises.
The exercise classes are supervised by registered nurses and exercise physiologists; they will monitor your blood pressure and EKG.
A home exercise program will also be developed to meet your physical ability, needs and interests.
Education
Individual counseling and education classes are provided to help you to understand and cope with your heart condition, and to understand the factors that contributed to your heart problems.
Behavioral
Our staff will help you develop skills to change unhealthy lifestyle habits and learn new behaviors that can reduce your risk for future health problems.
The weekly classes cover a variety of topics from nutritional eating to cravings, medications and the emotional aspect of heart disease.
Patients receive a comprehensive education manual, which serves as a valuable reference once they are on their own.
