Scheduling a Mammogram

There are two easy options to schedule your mammogram:

When scheduling, please verify the date of your last mammogram. Most insurances will cover an annual routine mammogram once every 365 days.

If cost or lack of health insurance keeps you from getting a mammogram, contact our Community Healthcare Access Team. They will see if you qualify for community programs or financial aid to cover the cost.

Mammography Patient Forms

We've made our forms available online so you can download and complete them prior to your appointment.

If your most recent mammograms or breast ultrasounds were performed elsewhere, we will need to get those scans at least two weeks prior to your EvergreenHealth appointment.

Download this form and send to the imaging center where you had your more recent mammogram or breast ultrasound.

What to Expect at Your Mammogram

In this edition of Check-Up Chat, Dr. Tamuna Chabra explains the importance of regular breast exam. She shares at what age you should schedule your first screening, what to expect during a mammogram, and how often you should schedule this very important preventive screening.
