Just as you may be referred to a cardiologist for heart care, you may be referred to Geriatric Care for help to improve your function, enhance your quality of life and maintain your independence.
How Geriatric Care Can Help
Serving ages 65+
You must be a resident of King County Public Hospital District No. 2, which includes Bothell, Duvall, Kenmore, Kirkland, Redmond, Sammamish and Woodinville.
Our experienced geriatric team will assess your medical conditions, cognitive status, functional ability and mental health needs to determine the best care for you.
Then we'll work in partnership with you, your family and your other health care providers to put a care plan into place.
We also work with hospitalized older adults, and we're able to reduce their hospital readmission rates because we partner with them, their families, their primary care physicians and other medical specialists to ensure a smooth transition from hospitalization to recovery.

Comprehensive Assessments
We evaluate your health status, including functional and cognitive ability, emotional health and your support needs.
Fall Assessments
If you have repeated falls, this evaluation and intervention planning will help reduce your risks.

Care Coordination
We coordinate your care needs with your primary care physician, specialty physicians, home care providers and other community resources.
Education
We work with you and your family to better understand topics such as living with chronic diseases, managing difficult behaviors and social problems associated with dementia, and making thoughtful end-of-life decisions.
Memory Loss and Dementia Assessment
There are many causes of memory loss, and it's important to know if yours is normal memory changes related to aging or memory loss associated with Alzheimer's disease or other forms of dementia.
Our comprehensive evaluation can help determine the cause, provide treatment and support you and your family.
The evaluation and expert diagnosis by our team is then used to develop your treatment plan.
