Physician Assistance Program

Here to help 24/7

First Choice Health provides the Physician Assistance Program (PAP) for you and your family. PAP services are cost-free, convenient and confidential; coverage includes a spouse or live-in partner and eligible dependent children. The service is available to all members of the medical staff, employed or independent.

You can reach PAP 24 hours per day, 7 days a week, at 800.777.1323 or online at www.fchn.com/Members/PAP (user name: evergreenpap).

When you call, you'll be greeted by a customer service professional ready to respond to your questions, needs and preferences. A mental health counselor is always available to speak with you or your family members.

The assessment providers in PAP's robust network are licensed mental health counselors, psychologists, clinical social workers, marriage and family therapists and chemical dependency professionals.

Services are confidential

We know that making it possible to consult discreetly with a behavioral healthcare professional is essential—perhaps the most important role of the Physician Assistance Program. Rest assured that information about your contact with PAP is never released without your request and signed consent.

Assessment & referral

A counselor will work with you to determine the nature of your concern and to resolve the issue within the available PAP visits. The counselor will also make recommendations about the value of treatment after the assessment in response to your unique needs.

Legal consultation

You have access to a cost-free 30-minute legal consultation, face-to-face or by telephone. Consultation is available as many times in a year as you have unrelated legal concerns. Typical problems include family and domestic concerns, motor vehicle matters, civil issues, elder law, divorce and parenting, juvenile issues and a variety of other concerns that are not work related.

Should you decide to retain the attorney for ongoing services, you will receive a 25% reduction in the attorney's normal hourly fees.

Eldercare services

If you're the primary caregiver or decision maker for an aging love one, the PAP can connect you to world-class eldercare expertise and resources - regardless of where your family member lives. From understanding Medicare to information about in-home nursing, assisted living and specialty health care providers, this comprehensive service can save you time and increase your peace of mind.

This service is provided at no cost to you or your immediate family.

Financial services

The PAP makes no-cost financial counselors available for consultation and education. Use this service as many times as you have unrelated financial concerns in a year. The consultation includes 30-60 minutes with financial professionals who will respond quickly to questions on topics including: budgeting, buying cars, estate planning, finding a mortgage, savings and investing, debt management, retirement planning and more.

Identity theft resolution

The average consumer spends over 175 hours responding to and resolving instances of identity theft. Our fraud resolution specialists are qualified legal professionals providing step-by-step guidance and consultation about restoring the integrity of your information and protecting your resources.

Personal/family mediation services

Similar to legal services, consultation with a qualified mediation professional is available to employed families. Typical concerns include decisions related to parenting plans, child custody agreements, child support, estate or property questions and a variety of other civil or family disputes or uncertainties.
