Volunteer Application

Please read the information under Steps to Becoming a Hospital Volunteer before filling out the Volunteer Application to ensure you have everything you need to complete the application.

It is especially important that you have all of your health documentation, as volunteer applicants will not move forward in the application process until we have received all of the needed immunization and TB confirmation.

Hospital Volunteer applicants will not be able to move forward in the application process until we receive all of the needed Immunization and TB confirmation.

We cannot accept partial submissions, so please make sure you have all of the documentation needed before you apply.

How to Submit Your Immunization Records

Make sure to clearly mark the Volunteer Applicant's name on each page.

• Upload records files where indicated on your online application (please use file names without spaces: "immunization-form.pdf" instead of "immunization form.pdf")
• Fax records to Volunteer Services: (425) 899-1997 Attn: Volunteer Services.
• Have your doctor's office fax records to Volunteer Services: (425) 899-1997 Attn: Volunteer Services.
• Send via US Mail to: Volunteer Services, EvergreenHealth MS 73, 12040 NE 128th Street, Kirkland, WA 98034.
