Student Placement Inquiries

Seek the support and guidance you need during your educational courses with a placement at EvergreenHealth. Here you will get supervised hands-on experience with leading experts in your field of study and see what your day-to-day will look like once you set out on your career.

Rotations are an important step on your academic journey. We'll help you take that step with confidence and offer the practical knowledge you'll need when you enter the field on your own. While we strive to accommodate everyone's request, we cannot guarantee we will be able to find a placement for every student.

What you'll need:

• Student's name, student's school name, placement coordinator's name and email address, start and end date of the placement, level of education given by the student's program and whether or not the student will be requesting a vaccine exemption.

If the placement has been arranged with someone employed at EvergreenHealth, please provide their name along with their department, clinic or unit.

For ARNP/DNP/MSN clinical placements:

• If the placement has been arranged, please provide the name of the clinic manager and the name of the preceptor's medical director unless the placement is in Primary Care.
• For placement requests, the student must be an EvergreenHealth employee. You'll also need the following information: the type of program, the number of clinical hours needed, preceptor requirements and the student's level of progress in their program.
• ARNP/DNP/MSN students must complete and submit the EvergreenHealth Clinical Inquiry Project form to StudentPlacement@evergreenhealthcare.org and will need approval from the Director of Education as well as the Research Department prior to starting.
• In general, the Student Placement Office will make an effort to arrange ARNP/DNP/MSN student placements for our employees. However, we cannot promise to do so. We will notify you if we are able to find a placement for the student.
• If the start date for this placement is more than three to four months away, please resubmit this form no more than 90 - 120 days prior to your requested start date. No wait list will be kept.

The form normally takes up to 15 minutes to complete.
