Use the Central entrance and park free in the Central garage.

Finding the Office

If you park on the 4th floor of the Central garage, you can enter the EvergreenHealth Surgery & Physicians Pavilion directly and follow signs to Tan 400.

Otherwise, take the elevator to the ground floor and walk to the EvergreenHealth Surgery & Physicians Pavilion entrance. Take the lobby elevator to the 4th floor and follow signage to Tan 400.

Appointments

We screen all incoming referrals to determine urgent and routine appointments.

A referral from another physician is required even if your insurance does not require it.

Before scheduling an appointment, we ask that all patients check with their insurance carrier to determine if EvergreenHealth Diabetes and Endocrinology Care is considered in-network or out-of-network.

If considered out-of-network, all patients (new and established) must furnish a referral from their primary care provider for their office visit to be covered.

Virtual Visits

We offer virtual visits for certain appointment types.

Late Arrivals

We try to accommodate all of our patients and understand that circumstances out of your control can make you late. However, out of respect for patients who are on time, you may be asked to reschedule.

Appointment Cancellations

Provide us with at least a 24-hour notice if you are unable to keep your appointment.

When should I arrive for my appointment?

If you are a new patient, arrive 30 minutes before your appointment. If you’ve been to our office within the last year, arrive at least 15 minutes before your appointment.

What should I bring?

  • Copay
  • Insurance card
  • Medical records that relate to your visit
  • Medication list or containers (if you have more than five medications)
  • Photo identification
  • If your address has changed, print and complete the Patient Registration (PDF) form.

Prescription Refills

When you need a medication refill, call your pharmacy. Plan ahead since refill requests take two business days.

Our Certified Nutritionist/Registered Dietitian will follow up with you regarding diabetes management, nutrition and diet and provides educational and training classes if needed for certain procedures and medications.


Quality Survey

Following your visit, you may receive a quality survey by email or by US mail from Press Ganey (a research firm). Patients are randomly selected. If you receive a survey, and complete it, we’ll use your feedback to help provide excellent patient care.
