If you have not completed your interview and health clearance for a hospital-based volunteer position:

• Please see the Volunteer Application and information. When you have completed the interview and received your health clearance, you will be directed to complete the Volunteer Orientation.

Please complete these parts to the Hospital-Based Volunteer Orientation:

• Annual Volunteer Policies (PDF) (print and sign the last page)
• Testing (read this information (PDF)), then complete our online competency exams)
• Agreement Packet (PDF) (print and sign all 6 pages)
