Hospital Volunteer Application

Please read the information under the Hospital Volunteer Application Process before filling out the Volunteer Application to ensure you have everything you need to complete the application.

It is especially important that you have all of your health documentation, as volunteer applicants will not move forward in the application process until we have received all of the needed immunizations and TB confirmation.

How to Submit Your Immunization Records

Make sure to clearly mark the Volunteer Applicant's name on each page.

• Have your doctor's office fax records to Volunteer Services: (425) 899-1997 Attn: Volunteer Services.
• Send via US Mail to: Volunteer Services, EvergreenHealth MS 73, 12040 NE 128th Street, Kirkland, WA 98034.
