The essentials of the model are organized in 3 overlapping circles, each representing an important part of the care we provide while holding patients and families at its core.
Center Circle: EvergreenHealth's values guide our nurses' relationships with our patients and other care providers by demonstrating our five values.
Inner Circle: These items are based on Jean Watson's 10 Carative Factors, guiding our behaviors of putting care into action.
Outer Circle: This represents the key elements of the Nursing Professional Practice at all levels of nursing at EvergreenHealth. These include innovators in care, leadership, professional development and evidence-based practice, research and relationships. The three important relationships are with the self, the team and the patient and their family. A good relationship with the self leads to a good relationship with the team, which leads to synergy in the relationship with the patient and family.
