EvergreenHealth Monroe will strive to efficiently and effectively carry out its responsibilities under RCW Chapter 42.56 – the Public Records Act.

If you wish to request access to public records of EvergreenHealth, Monroe or seek assistance in making such a request, you should follow EvergreenHealth Monroe's Public Record Policy.

Public records requests must be submitted in writing, and must include the following information:

  • Your name
  • Your address
  • Your phone number
  • Your email address
  • Date and time of the request
  • Enough identification of the public record(s) to enable the Public Records Officer to locate it.

Exemptions

The Public Records Act provides that a number of types of documents are exempt from disclosure. In addition, documents are exempt if any "other statute" exempts or prohibits disclosure. Requestors should be aware of the following exemptions, outside the Public Records Act, that restrict the availability of some documents held by the District:

RCW 42.30.110(1)(o) – Executive sessions

RCW 70.02.270 – Medical Records

45 CFR § 160-164 – HIPAA privacy rule


Fees

If you wish to get a copy of the records rather than just inspect them, you'll need to make arrangements with the Public Records Officer to pay for copies or scans.
