Public Notice of Board Nominees

November 15, 2024

On November 14, 2024, EvergreenHealth Monroe held a public meeting to select nominees to be considered for Commissioner Position #5, vacated by Commissioner Matuschak on October 21, 2024.

As required by RCW 42.12.070 (as amended by SB 5437-S.SL) the Board of Commissioners hereby notifies the public that the following individuals are nominees for this position and will be interviewed in a future open public meeting:

  • Anthony Balk
  • E. Dianne Forth
  • Florence Martin
  • Judith Kuleta

This notice will be posted for 15 days, through November 29, 2024, and is also posted in the following locations at EvergreenHealth Monroe located at, 14701 179th Ave SE, Monroe, WA 98272:

  • Public Meeting Bulletin Board (located by the Godard Room entrance)
  • Administration

Registered King County voters who reside within Snohomish County Public Hospital District #1 boundaries** may submit nominations (or self-nominate) by completing the Commissioner Nominee form and submitting it by November 29, 2024. 

Board Member Qualifications

Nominees must, at a minimum, meet the qualifications outlined below.

  • Be a resident of Snohomish County Public Hospital District #1 (SCPHD#1)**
  • Be a registered Snohomish County voter
  • Complete a disclosure of conflicts of interest
  • Not be employed by SCPHD#1
  • Be willing to provide public disclosure per RCW 42.56 Public Records Act (e.g., Personal Financial Affairs Disclosure (F-1))
  • Adhere to all applicable state laws, including those pertaining to Public Hospital Districts (RCW 70.44), the Open Public Meetings Act (RCW 42.30), Public Records Act (RCW 42.56), and Code of Ethics for Municipal Officers (RCW 42.23); and adhere to all applicable federal laws, including Centers for Medicare & Medicaid Services regulations.
  • Adhere to EvergreenHealth's vaccination requirements
